ROOM 8 SLEEP CLINIC β€” Clinical Audit Floor Briefing

10 subjects admitted overnight. Audit in pairs. Mr. Waugh • Bicentennial Junior High

AUDIT IN
PROGRESS
The Deal & Audit Format

Ten Grade 9 students were audited for 24 hours: schedules, devices, intake, and field observations β€” nothing else. You and your partner are consulting clinical auditors. Work in pairs to investigate the 10 subject stations posted around Room 8. You may visit the stations in any order, at your own pace. At every station, complete one row of your Response Sheet (Deliverable 19). Leave no graffiti on the subject files β€” these are patients, not suspects.

At Every Station (5 steps)

  • β‘  Sleep hours: onset β†’ wake. Read it off the log β€” subtract carefully.
  • β‘‘ Weekly debt: (9.0 βˆ’ hrs) Γ— 5 school nights.
  • β‘’ Risk code: [STABLE] / [MODERATE] / [CRITICAL] using the scale.
  • β‘£ Mechanism codes: tag every mechanism that is live, using the code bank on Side B of your sheet.
  • β‘€ One-line order: the single change you would prescribe first.

Risk Scale (Greyscale B&W)

βšͺ STABLE (GREEN)β‰₯ 8.5 hrs AND clock stable (weekend shift < 1 hr) AND zero live mechanisms.
πŸ”˜ MODERATE (AMBER)7.0–8.4 hrs average, or hours fine but clock shifted > 2 hrs, or exactly one live mechanism.
⚫ CRITICAL (RED)< 7.0 hrs average, or multiple live mechanisms, or fragmented sleep.

⚠️ Not every subject is a disaster. At least one of these kids is fine. "Maintain and protect" is a legitimate order β€” if the file earns it.

Auditor's Rules of Evidence

  • Diagnose from the evidence β€” times, device logs, quotes β€” not from stereotypes.
  • Time in bed is not time asleep. Check for onset delays and night wakings.
  • Hours are not the only variable. A clock that shifts > 2 hrs on weekends is broken even with full hours.
  • If the hygiene is already clean, do not prescribe hygiene. Read again.
OPEN CLINIC PROTOCOL: Work with your partner at your own pace. You do not need to follow a rigid rotation chime. Visit the 10 stations in any order. Prioritize completing at least 6–8 triage audits (or all 10). Once the clinic audit period concludes, your firm will select ONE subject to write a full clinical audit report using the Google Docs / Chromebook template before Quiz 1.
